I don't know if there is a standard translation for it, but I think this is correct

4 KudoZ points awarded for this answer. Comment: "Thank you all for answers. I did not think "custody", (if it is a US specific term), was appropriate here, since the client was based in Europe. "Guardianship" also did not work for me, as the ”親権者" in this case was the child's actual parent. Thus,"Parental Authority" seemed to be the "safest" term to use. Thanks again! "

The date fixed the person with parental authority

親権者=person with parental authority.
As I am a court interpreter for the State of Hawaii, I use this term quite often.

Date of the Establishment of Guardianship

Guardianship can be used for people of for estates. I think that this term is better for a birth certificate.

This was taken from an online dictionary of legal terms.

The prefered term might be different for British and American English. This the term I see in the U.S.

Date of Parental Custody Designation

I translate Japanese family registers which is used as a requirement for those who apply for local house loans in the Philippines and I use the word "Custody" for the reason that the Philippines uses American English. Hope this helps!
