Assistant Attending vs. Associate Attending vs. Attending

English Other Medical: Health Care fractures through bone cysts
e.g.
Associate Attending Physician
or
Assistant Attending Surgeon
or
Associate Attending Psychologist


It is my understanding that these are stages of promotion of a healthcare professionals (usually M.D., but not always, e.g. Associate Attending Psychologist may not have M.D. degree) within their institution; but what exactly is the difference between these positions? When is one promoted from "assistant attending" to "associate attending" or from "associate attending" to "attending"? What are the common criteria for each of these positions?

To Yang Min Please read my question again. I knew the precedence of these three before I asked this question; what I need is answer to these questions:
1a. when assistant attending is promoted to associate attending?
1b. when associate attending is promoted to attending?
2. for a new employee, what criteria will institution use when assigning him one of these three ranks?

Yes, Rita, but they still may be "Assistant/Associate Attendings", and that is pretty much confusing; the actual text that initially raised this question featured "Associate Attending Psychologist", who works in a medical institution and has Ph.D.
