There is really no other context. "Responsable" is where the person signs. I understand what they mean more or less but am not sure what the official term would be in English. Any thoughts?

: I have to agree with Henry here as there is no more context I would say "signed by: ...". I would have opted for Terry's suggestion but more context would be needed.
