There is really no other context. "Responsable" is where the person signs. I understand what they mean more or less but am not sure what the official term would be in English. Any thoughts?

Precisely, if I knew who signed, I probably wouldn't have had a problem with the translation. As I said, there is no more context than already given - that was the reason for my question.

With no indication of who this is, you cannot guess and give any specific term; therefore with no other CONTEXT, the only option is "BY" because it is completely neutral.

agree Sandra Holt : I have to agree with Henry here as there is no more context I would say "signed by: ...". I would have opted for Terry's suggestion but more context would be needed.
