As this is a psychology question, I was going to suggest "staring into
the void", but this doesn't apply here (that's more Philosophy!)

A suggestion - an expression which avoids the spaced out connotations, as you request. Per se staring into space doesn't actually carry this connotation, imo, but in this context I agree that it may best to avoid the expression!

This wording, adding the word 'off', is the best way I can think of to indicate that a person's attention is absent, or focused away from reality.
The complete translation would be: "the patient appears to be staring off into space"

It'd just that, on its own (w/o more context) "concentrating on listening" seems a little odd... though not impossible!
