Discussion

slothm Oct 23, 2008:
No doubt now. A little bit of context may have helped.
However, we now know that there is a DEP creatinine evaluation test.
This test is mandatory for exposed persons like pesticide sprayers, formulation developers, etc.
Roxanna Delgado Oct 23, 2008:
Thanks for the clarification. I guess the correct answer it's pretty clear now. Just the usual pre-op lab tests.
Janet Ross Snyder (asker) Oct 23, 2008:
pesticide exposure unknown The purpose of the physical examination was to determine if an endovascular aneurism repair (EVAR) should be done and if the patient was fit to undergo surgery. The patient had had hip surgery and a bypass, but no history of alcohol abuse or poisoning of any other kind was mentioned in the history.
Roxanna Delgado Oct 23, 2008:
Hi jlrsnyder. We really need you to clarify the mistery for us. Was this patient exposed to pesticides or not?
Rita Tepper Oct 23, 2008:
slothm Oct 23, 2008:
There are two interpretations,
1.- Renal Clearance.
2.- The sample was obtained from a person exposed to OP pesticides. DEP is the more common metabolite of OP poisoning and meassures the rate of detoxification. It is expresed as creatinine

disagree slothm : Please note Rita that the list of results has two creatinine determinations, The first one is "creatinine" and the second is "DEP creatinine". Your suggestion applies to the first one.

slothm, DEP goes for "depuración" de creatinina, the spanish translation for "clearance", though it is frequent to use the English term "clearance de creatinina"
agree David Shield : Yes, creatinine clearance makes sense, especially since the units reflect a RATE, not a simple quantity. From "depuración de creatinina".

DEP creatinine

DEP is diethylphosphate, a metabolite of organophosphorous intoxication (pesticides) to be found in the urine as a metabolite after passage through the liver.
The real creatinine level in blood must be corrected due to the presence of DEP (upper case).

Creatinine Clearance

Many laboratories nowadays calculaute the creatinine clearance or Glomerular Filtration Rate only using the plasma creatinine, using the Cockcroft- Gault Formula, an example of which is given here
