under / above: these words can be a bit misleading or confusing sometimes. It depends whether the patient is standing or lying down.
Perhaps it's best to stick to "within".

The normal liver is 'within' or 'above' the costal margin:

The normal upper level of the liver is at the level of the right nipple, whereas the lower margin of the liver is at the right costal margin.

liver, spleen within the rib cage

in Czech, it means "v žeberním oblouku"
liver should be normally located within the rib cage I believe
