Could it be "destar" as an unofficial abbreviation for "destacar"? I mean, SYNDROME is Spanglish or English in the middle there, so the doc was typed poorly, in a rush, etc.
Just a guess...
Most Goggle hits were of "de estar", improperly written, in some form, as in your reference, and that does not fit with the rest of the diagnosis as written.

I am going to propose something different with respect to D Estar. The term 'hipertensión arterial-D I take it as "High Blood Pressure and Diabetes." A low confidence level appropriately goes with such proposition.
