The second set of letters looks like Ruidos Cardiacos Ritmicos y Regulares ... the first one, I'm guessing, has to do with Murmullo Vesicular ... Campos Pulmonares ... right, Neil ... I agree with what you just wrote.

agree Charles Davis : You missed out the first Ps, which I think means "presentes", so it doesn't make any practical difference :-)

Yeah, it's a bit wordy. I just chart it as SDP (sin datos patologicos) in my reports. I always tell medical students that notes should be brief and concise. Using overly long phrases or poor handwriting is a sign of obfuscation.
