English to German Law/Patents Law (general) Statutory Declaration - Australia

All,

I have a job translating several statements of a client that they intend to make in front of an Australian Notary Public.
My client has written the statements with spaces for date and signatures of the declarant and the Notary Public.
The have also added the following certification statement (to be made by the Notary Public).

Declared at Sydney on {space} [date] {space} [signature of declarant].

In the presence of an authorised witness, who states:
I, {space}, an Australian Notary Public, certify ***the following matters*** concerning the making of this statutory declaration by the person who made it.

{space} [signature of authorised witness] {space} [date]

****

I am little lost with understanding the bit "… the following matters concerning the making of this statutory declaration…".
A colleague has told me that the "following matters" concern how the identity of the person making the stat dec was established.

So far, I have "Ich, … beglaubige die nachfolgende eidesstattliche Erklärung durch die Person, die sie gemacht hat." but I need to work in the "following matters".
