I'm wondering whether this term means "referral hospital" or "referring hospital" but am a little confused about the exact system. Can anybody help please?

This would be an option.

E.g.,

* notify the sending hospital of the patient’s safe arrival and placement

Since it refers to which hospital the patient is coming from, I would write "from xxx hospital" or maybe even "From Hospital: XXX".
I could not find any links or sample sentences to support this, so let's see what the others think.

However, this would be more appropriate if the patient was treated in the hospital as inpatient and now reading the question again 'patient included in a clinical trial on an outpatient basis', I am not sure if this applies here.
