referring hospital
Referring hospitals often provide diagnostic and therapeutic equipment for adequate primary care of stroke patients, but usually do not have dedicated stroke experts on staff. Telemedicinal equipment can compensate for this lack of critical medical expertise. With the help of a neurologist in a consulting hospital, this may allow for accurate diagnosis and early treatment in nonspecialized clinical environments, thus improving the prognosis of stroke patients substantially…. It enables the integration of small clinics' hospital IT with specialized centers, making diagnostic evaluation and treatment recommendations by stroke experts available at the referring hospital.

Collaboration between a referring hospital and a tertiary care center in improving the transfer process for cardiac patients.
Ongoing discussion between the cardiology team at the accepting hospital and physicians at the referring hospital expanded the continuum of care to both hospitals. Priority of transfers could be upgraded at any time without unduly jeopardizing patient safety or increasing resource utilization at the receiving center.

sending hospital
This would be an option.
E.g.,
* notify the sending hospital of the patient’s safe arrival and placement

from xxx hospital / from hospital: xxx
Since it refers to which hospital the patient is coming from, I would write "from xxx hospital" or maybe even "From Hospital: XXX".
I could not find any links or sample sentences to support this, so let's see what the others think.

transferring hospital
It is also common to find 'transferring hospital'
With the exception of GI Bleed, patients more often received specialty procedural care at the receiving than the transferring hospital.

The transferring hospital must send copies of all medical records related to the emergency medical condition

However, this would be more appropriate if the patient was treated in the hospital as inpatient and now reading the question again 'patient included in a clinical trial on an outpatient basis', I am not sure if this applies here.
