The scope and the retractor were inserted through the incision for subcutaneous dissection. The surgeon used his left index finger as a fulcrum, so that the retractor could be held easily with only one hand. At the beginning of the procedure, the previous implant was left in place to provide a counter-traction force for separating the plane between the capsule and the overlying tissues.
