Assuming that T. stands for "type".
I initially thought it stood for "tarjeta" (card), but "type" seems more likely.

One problem is the fact that "Asistencia" can mean support/assistance as well as attendance, so without further information, it's difficult to pinpoint the exact meaning here (without further context).
What I find interesting is the fact that they use "CLIENTES" rather than "PACIENTES"... which suggests a more business-oriented approach rather than medical...

A Google search for "attendance type" gets roughly 66,000 hits, whereas "assistance type" gets over four hundred thousand, although this only gives you an idea of usage frequency of the two expressions.
