As an ophthalmologist here is my explanation:
During cataract surgery with phaco technique, we first make a main clear corneal incision in the form of a tunnel (in this way it will have self-seal) for inserting the handpiece and another one narrow side incision at a 60 or 90-degree angle to the tunnel for manipulation with other surgical instruments - if needed two side-port incisions can be made.
At the end of the surgery we leave these wounds sutureless, however if we see a risk of leakage we put a 10/0 nylon suture to the tunnel incision, less likely to the side-port incision.
So for "puerto principal" you can use "main incision" or "tunnel incision" and the "acesorios" would be the "side-port incisions".

Given the texts quoted below, I'm pretty sure "port" is correct, although the "accessories" might be better translated as auxiliary or secondary.
