"Mestparten av tumor inklusiv kontrastoppladende del fjernet".

Does anyone have any ideas what the "kontrastoppladende del" could mean?

Meningeomene utgjør 25 % av de primære intrakraniale svulstene. Forekomsten øker jevnt med alderen, med overvekt av kvinner (kvinne/mann = 1,7/1). Kvinner som har fått diagnostisert et meningeom, har en lett overhyppighet av brystkreft og vice-versa, antakelig fordi begge svulstformer er påvirkelige av kvinnelige kjønnshormoner. Meningeomene oppstår fra araknoidale celler og er hos 95 % av pasientene godartete. Radiolgisk ser man typisk en homogent kontrastoppladende svulst med bred anleggsflate mot dura/skallen og med god avgrensning mot hjernevevet. Dura er imidlertid vanligvis infiltrert av svulsten, av og til også tilgrensende deler av kraniet. De fleste meningeomer har et arachnoidalt lag mellom seg og hjernen og infiltrer ikke hjernen, men skyver den foran seg under veksten. De kan imidlertid vokse rundt blodkar og hjernenerver. Dette skjer særlig når svulsten vokser ut fra skallebasis, og den kan da være svært vanskelig eller umulig å fjerne. Meningeomene vokser vanligvis svært langsomt. Hjernen kan derfor adaptere seg til svulstens vekst, noe som kan forklare hvorfor pasientene kan ha appelsinstore svulster før de får så klare symptomer at tilstanden diagnostiseres. Atypiske og anaplastiske meningeomer har et mer malignt vekstmønster enn de vanlige meningeomene, og de har derfor en betydelig dårligere prognose.

Prognosen for pasienter med hjernemetastaser avhenger blant annet av histologisk type, grunnsykdommens utbredelse, nevrologisk funksjonsstatus og antall hjernemetastaser. Antall hjernemetastaser vurderes på MR, og i dag anslår man at 25 % av hjernemetastasene er solitære, mens 75 % er multiple. På MR er hjernemetastasene som regel runde, velavgrensede, homogent utseende, kontrastoppladende og omgitt av et kraftig ødem.

I dagens sykehuspraksis er CT-undersøkelse førstevalget ved primærutredning av
pasienter med redusert mental status, mistenkt hjerneslag og intrakranial blødning samt
ved hodetraume . CT med kontrast brukes til å diagnostisere hjernesvulster, men har klare
begrensninger i forhold til MR siden ikke-kontrastoppladende svulster lett kan bli
oversett. MR er mer sensitiv enn CT for å påvise patologiske prosesser i hjernen, spesielt
ved undersøkelser av bakre skallegrop og strukturer ved skallebasis (8). Man unngår (i
motsetning til CT) bildeartefakter fra benstrukturene. Det er også vesentlig at man med
MR ikke utsetter pasienten for ioniserende stråling. Begrenset tilgang til
MR-undersøkelser (spesielt på vakttid) medfører at primærundersøkelsen ofte er CT.
Dette har imidlertid endret seg de siste årene med stadig økt utbredelse av MR-maskiner
til de enkelte helseregioner.
