"Spalt" refers to a thin space between the dura and the arachnoid. This is not normal, but due to disease or trauma.
In this case, I assume that the space had been observed in some radiographic modality, and the patient was then sent to Lund to get it punctured.
I believe that the most common way to describe it in English would be to call it a "subdural fluid collection", but as the author has used "spalt" I guess you could translate it as "space"
