neutral Muriel Vasconcellos : I'm not so sure. That would yield a translation along the lines of "a clinically caused adverse event" - i.e., an iatrogenic event. I don't think that's what they're talking about.

This may not be the best wording, but the distinction is between what we would call an elective procedure and a condition arising at the clinical level that has progressed to the point that it needs to be referred to a higher level of complexity in the health system.
